  1. Proof of insurance along with a certificate of insurance (COI) naming Spring Kitchen Co.
  2. Permit from Arkansas Department of Health
    • This may require a notarized permission letter from Spring Kitchen, which we will be happy to provide
  3. Proof of Business License
  4. Manager Serv Safe Certificate
    • Serv Safe can be obtained from the following locations:
      • Northwest Arkansas Community College (NWACC)
      • Brightwater
      • Fayetteville Public Library
